Submit (electronic, typed or hand-written copy), nomination to Linda Ahuna-Hamill, 201 Administration Building. All nominations will be acknowledged upon receipt. For full consideration, nominations must be received by 5:00 p.m. Thursday, May 14, 2009.

This award will be given to a person who has demonstrated a commitment to the following:

  • Providing quality service to student.
  • Willingness to go "above and beyond" to reach identified goals.
  • Helpfulness, professionalism and assistance.
  • Makes a positive difference in the work environment and demonstrates a commitment to diversity.
  • Consistently demonstrates quality service to the University community.
  • What other information do you think is relevant to the applicant's consideration for the Quality Service Award?
